Sesamoid bones are small bones consistently found within the joint of the great toe, crucial for protecting the major tendons and serving as a fulcrum for forward propulsion during walking. Due to their location and the extreme forces they absorb, they are highly susceptible to injury, including fractures and inflammation known as Sesamoiditis. Sesamoiditis is an inflammatory condition of the bone lining, typically caused by excessive activity, running, ballet dancing, wearing high heels, or having a high-arched foot type.

X-rays are crucial to differentiate between sesamoiditis and a true sesamoid fracture, as treatment is dramatically different. Because a high percentage of people have naturally multipartite (fragmented) sesamoids, a bone scan or MRI may be necessary to confirm a fracture versus a normal variant.
Since sesamoiditis is an inflammatory condition, treatment is aimed at reducing inflammation, which may include rest, ice, anti-inflammatory medications, and physical therapy. More resistant cases may benefit from a cam-walker removable cast or occasional cortisone injection (performed only after ruling out a fracture). Conversely, sesamoid fractures require a more aggressive course of treatment, typically involving cast immobilization for 6-8 weeks. Long-term therapy for both conditions involves identifying and modifying the underlying cause, which often includes the use of custom orthotic devices, calf muscle stretching, or shoe modifications.
